Certificate of Origin

Certificate of Origin Procedures

A Certificate of Origin is an export document certifying that the referenced goods were produced in the United States. Greater Louisville Inc. – the metro chamber of commerce is pleased to provide this service to our members as well as non-members. Below is a description of GLI’s certification process and links to helpful resources.

  • Complete, sign and have notarized the Certificate of Origin.
  • Complete, sign and have notarized the Greater Louisville Inc. Affidavit of Certificate of Origin.
  • Bring notarized documents along with the bill of lading or other shipping documents such as an airbill / waybill, plus an invoice if possible.
  • You must provide us with a duplicate copy.
  • Payment is required at time of service.

Certificate of Origin Forms

You may obtain a Certificate of Origin form online or create your own form based on existing online forms. If creating your own form, the following liability statement must be included in certificates of origin for commodities of United States origin, for authentication by Greater Louisville Inc:

The Louisville Area Chamber of Commerce, a corporation organized under the laws of the State of Kentucky, does hereby certify  that to the best of its knowledge and belief the statements made herein concerning both prices and country of origin, are true and correct.  

If you are using a standard form, it will be stamped with the above language when you visit our office.

Affidavit for Certificate of Origin

This affidavit releases GLI from any liability with regards to the origination of the goods. Certificate of Origin Affidavit

Notarization

The Certificate of Origin and the Affidavit must be notarized before GLI will certify the documents. Notarizations can be done at GLI for a $15 fee for non-investors. The fee is waived for GLI investors.

Accompanying Documentation

Please include a bill of lading, or other shipping documents such as an airbill / waybill or a commercial invoice, if possible. A duplicate copy of all documents must be provided for our records.

Payment

GLI investors, $15 per visit for certificates of origin

Non-investors, $100 per visit for certificates of origin

Notary Services
GLI offers all Notary Services, free of charge for investors; non-investors are charged a $15 fee.
